for Transported Semen |

I, _____________________________, hereby confirm my reservation to the
stallion named Dynamic for the _________ breeding season. If said stallion is alive,
fit to breed and standing at Affinity Sporthorse Farms, I understand and accept the
following terms: I have read the fee schedule and have chosen to breed to the
stallion Dynamic using Fee Schedule # 1
2 3 . The total fee for breeding to this stallion,
using this fee structure, will be $_____________ (includes a booking fee of $250.00 which
is non-refundable). A deposit of $500.00 to cover container, collection fees and
shipping costs is required. The unused portion of this deposit will be returned to
the mare owner or applied toward the balance of the breeding fee when the mare is declared
in foal.
1. A veterinary certificate and current uterine culture taken
when the mare is in heat and submitted to a recognized laboratory, showing the mare to be
in sound breeding condition and free of infection, is a condition for the service.
In the case of maiden mares, this may be waived. However, a note from a veterinary
indicating the mare is a maiden mare will be required in lieu of the certificate of
breeding soundness.
2. A $______________ non-refundable booking fee and a
$______________ semen shipping deposit must be returned with this contract. The
balance of the breeding fee, which is $_________________ , must be received by the
stallion owner when the mare is declared 45 days in foal. A breeding certificate
will be issued when the breeding fee is paid in full. I understand that my mare must
be examined by a licensed veterinarian 40 and 50 days after the last day bred, and the
stallion owner notified of the pregnancy status at that time, or all live foal guarantees
are null and void. I understand that the mare's breeding passport must be signed by
the veterinarian and returned to the stallion manager or owner or all guarantees are null
and void.
3. The stallion owner assumes responsibility to deliver live
semen in viable condition at the time of delivery to the mare owner's farm or to the
approved breeding station. This semen must be administered by a licensed
veterinarian or by a breeding technician approved by the stallion owner. This
approval constitutes a part of this contract and without it the live foal guarantee is
null and void. The semen must be administered the day of delivery or the stallion
owner shall be considered to have fulfilled the contractual obligations and any guarantees
in subsequent clauses shall not apply.
4. I understand that the veterinarian who is to perform the
insemination must contact White Oak Ranch manager (Pam) by telephone or in person, before
semen will be shipped. Failure to do so may absolve stallion owner of any
obligations under this contract. It is absolutely essential that communication be
established and maintained directly between the inseminating veterinarian and the breeding
manager. The success of the transported semen program depends upon close cooperation
between the stallion manager and the veterinarian managing the mare.
5. This contract guarantees return privileges for the same
mare during the course Affinity Farms' breeding season (March 15 - August 1). If
mare owner has subscribed to Fee Schedule #2, mare owner is guaranteed return privileges
for the season following the initial breeding; if mare owner has subscribed to Fee
Schedule #1, mare is entitled to return breeding privileges for three seasons following
the initial breeding. A second mare may be substituted with the permission of the
stallion owner. This contract is not transferable without written permission of the
stallion owner.
6. In the case of guaranteed return breeding rights, should
the mare not provide a live foal that stands and nurses, the mare owner shall be entitled
to return breedings. This guarantee shall only apply if the stallion owner is
notified by certified mail within 72 hours of the foal's death. A statement of
details by a licensed veterinarian must follow within ten days or the guarantee becomes
null and void. This clause is also binding in case of abortion. If the mare
reabsorbs her pregnancy, we need written verification of this fact from your veterinarian.
7. I understand that the mare owner shall assume all
responsibility for the condition of the mare and shall bear all risk of loss or damage to
the mare whether by death, disease, injury, infection or otherwise, and by any cause
whatsoever, and therefore agrees to hold the stallion owner harmless for any and all
damages associated therewith.
8. Should the stallion die or become unfit for service, this
contract is and shall become null and void. The determination as to the fitness
shall be made by the veterinarian regularly in attendance for Gayle Atkins.
9. Fees:
a) A non-refundable booking fee of
$_____________ and a "semen shipping" deposit of $________________ are to
be returned with this contract. The balance of the breeding fee is
due either before semen is shipped or when mare is declared 45 days in foal.
I have elected to pay the breeding fee in
full before semen is shipped and take advantage of the 10% discount.
The balance of $__________________ will be paid before my mare is bred.
I have elected to delay paying the balance
of the breeding fee until my mare is checked 45 days in foal.
The balance of $__________________ will be paid when my mare is checked in foal.
b) Mare owner will pay a $75.00 fee
per collection for semen which is shipped to their mare. If it is necessary to
rebreed the mare during the following breeding seasons, I
understand that the collection fee will be $85.00 per collection.
c) Mare owner understands that
requests for semen shipment must be made by 8:00pm of the day preceding the requested
semen shipment or the collection fee may be doubled. Please inform your
veterinarian of this time deadline so that they may schedule their examination
accordingly.
d) Fees for transportation and
collection will be deducted from the shipping deposit of $500.00. The remaining
unused deposit will be refunded or applied toward payment of the balance of the breeding
fee when mare is pregnant. If the collection and shipping costs exceed the deposit,
which is highly unlikely, a further deposit will be requested before more semen will be
shipped. Approximate transportation costs are: Federal Express, $40.00; Air
Lines Counter to Counter, $70.00; Courier Service to Airport, $45.00.
e) Semen shipping containers must be
returned within two days of receiving them. This is extremely important to ensure
adequate supply of shipping containers. Containers must be returned before semen
will be shipped for a subsequent heat. United Parcel 2nd Day is the best way of
returning containers.

Please be sure to: 1) Call us when your mare foals; 2) Call
us on the first day of your mare's heat cycle on which you wish to breed her; 3)
Have your veterinarian call us before we ship semen the first time; 4) Explain to
your veterinarian that we collect our stallions in the morning and that we need
notification by 8:00pm the night before we ship semen. When calling us to order a
shipment of semen or in reference to breeding, please call the Stallion Station at (541)

Stallion Service Contract - Fee Schedule
Fee Schedule #1:
This fee is $1,850. Mare owner is guaranteed return breeding
privileges for three seasons following the initial breeding. A fee of $250 will be
charged at the time of rebooking the return each subsequent season.
Fee Schedule #2:
This fee is $1,500. Mare owner is guaranteed return breeding
privileges for the season following the initial breeding. A fee of $250 will be
charged at the time of rebooking the return for the second season.
Fee Schedule #3:
This fee is $1,250. It must be paid prior to the first
shipment. This schedule guarantees return privileges for the same mare during the
course of Affinity Farms' current breeding season (March 15 - August 1).
**NOTE** A discount of 10% can be
deducted from these fees if the mare is booked prior to March 1st.
